Bruxism Treatment Guy answers:

I’m a dentist.

There are varying explanations for bruxism and none of them are particularly good.

With young children, bruxism is exceedingly common. I have heard that is has to do with a child’s occlusion not being completely developed. (Occlusion = bite). Just FYI, occlusion is an exceedingly complicated topic. So complicated, in fact, that the dental profession doesn’t understand its effects on the musculature of the head and neck very well. Theories abound, but there’s not much proof of anything.

In general, for most people, their masticatory muscles are in their most-relaxed state when the teeth are in their proper place with the cusps of the upper teeth in the pits of the lower teeth (without actually touching, mind you), and vice-versa. This is called “maximal intercuspation”. Any deviation from this, and the muscles supposedly become more active.

Hence, some doctors theorize that problems with the occlusion can promote bruxism. Indeed, I have experienced this myself when I was wearing Invisalign. The thickness of the plastic between my teeth prevented my teeth from intercuspating, and lo- and behold, I found myself constantly grinding on the Invisalign trays.

For children, supposedly their occlusion has not been developed, and thus their masticatory system doesn’t know what’s “comfortable” and where to relax. Hence, the grinding.

Interestingly, this all does make some sense. Your masticatory system (i.e. Teeth, jaws, muscles, TMJ, and the nerves that control it) is exquisitely sensitive! If I put a tiny pebble between your teeth, you’d be able to tell that your jaw muscles were slightly stretched out. Your bite consists of more than just where your teeth come together. Eventually, it seems as though that your jaw muscles and TMJ follow suit, and recognize your bite as where things are comfortable.

Of course, the TMJ itself plays a role. There is something we call “retruded position” or “centric relation”, which is a certain position of your TMJ that is supposedly the basis of your occlusion.

Like I said, occlusion is very complicated, and moreover, no one knows for sure that occlusion is a cause of bruxism.

Stress, medications, etc. Can promote bruxism, though.

There are two words that will make this possible to do or impossible to do:
These two words have a magic all their own –they are magical because they remind me of “Open Sesame” — once you use these words they open a new life for you if you apply them or close the door to it, they are called: Discipline and Dedication. With out these magical words nothing is possible—I will give you the path to a new life, but you must walk it and never get off of it — until the day that your real cause-point is found and eliminated.

TMJ Problems!!! Please help!?

I’m really not sure what to do about my TMJ. I had braces put on when I was about 12, and about a year later I started having bad TMJ. I’m 20 now. My orthodontist continued with my treatment but I have TMJ and an open bite (which I have always had) I was looking at getting at finding a doctor that will do an ALF appliance but I’m not sure. My open bite isn’t that bad, and I only hear a “click” noise when I move my jaw from side to side. It doesn’t click when I open, close, or eat. I really only experience facial pain and my ears have been feeling a little clogged, with minor ringing noises. I’m afraid that splint therapy will make my teeth crooked when I’ve already had 3 years of braces to straighten them. My TMJ is problem is only on the right side. Any thoughts on a treatment plan? I’m very against surgery F.Y.I.

Bruxism Treatment Guy answers:

In some cases, the symptoms of TMJ disorders may go away without treatment. If your symptoms persist, your doctor may recommend medications or a bite guard to help keep you from grinding your teeth at night. In very rare cases, surgery may be required to repair or replace the joint. I know you say you are against surgery, but my friend suffered from symptoms for years and finally had surgery and says she wished she had had the surgery years ago.

Medications

Painkillers. If over-the-counter pain medications aren’t enough to relieve TMJ pain, your doctor or dentist may prescribe stronger painkillers.

Tricyclic antidepressants. Antidepressants, such as amitriptyline or nortriptyline, taken at bedtime help relieve TMJ pain in some people.

Muscle relaxants. These types of drugs are sometimes used for a few days or weeks to help relieve pain caused by TMJ disorders.

Corticosteroid drugs. For significant pain and joint inflammation, corticosteroid drugs injected into the joint space may provide relief.

Botulinum toxin. Injecting botulinum toxin (Botox, others) into the jaw muscles used for chewing may relieve pain associated with TMJ disorders.

Therapies

Bite guard. If you grind your teeth in your sleep, you may benefit from wearing a soft or firm device inserted over your teeth. This bite guard prevents your teeth from meshing together. Bite guards sometimes aggravate sleep apnea symptoms.

Cognitive behavioral therapy. If your symptoms of TMJ disorder are made worse by poorly managed stress or anxiety, your doctor or dentist may refer you to a psychotherapist with experience in cognitive behavioral therapy. This approach includes interventions to help you be aware of and change behaviors, learn relaxation techniques, and manage stress.

Surgical or other procedures

Corrective dental treatment. Your dentist may improve your bite by balancing the biting surfaces of your teeth, replacing missing teeth, or replacing needed fillings or crowns. However, these types of treatments sometimes worsen TMJ pain.

Arthrocentesis. This procedure involves insertion of a needle into the joint so that fluid can be irrigated through the joint to remove debris and inflammatory byproducts.
Surgery. As a last resort, your doctor or dentist may suggest surgery to repair or replace the joint. But the National Institute of Dental and Craniofacial Research considers TMJ surgery to be controversial and recommends that it be avoided whenever possible.

Becoming more aware of tension-related habits — clenching your jaw, grinding your teeth or chewing pencils — will help you reduce their frequency. The following tips may help you alleviate symptoms of TMJ disorders.

Avoid overuse of jaw muscles

To minimize the use of jaw muscles:

Eat soft foods
Cut food into small pieces
Avoid sticky or chewy food
Avoid chewing gum
Don’t open your mouth too wide during yawns
Stretching and massage
Your doctor or dentist may show you how to do exercises that stretch your jaw muscles and how to massage the muscles yourself. He or she may also show you exercises to improve your head, neck and shoulder posture.

Heat or cold
Applying warm, moist heat or ice to the side of your face may help relax your muscles or alleviate pain.

You won’t be permitted to stay on them for long. Benzos are addictive tranquilizers and intended only for temporary relief. Pain from TMJ is something you’re going to have to learn to live with.

Bruxism Treatment Guy answers:

Don’t worry that’s not gonna fix the problem. The child MAY (I’m not a professional) be experiencing a terrible night dream or the child may have Worms/Parasites. For the dreams. Let the child sniff some kind of aromotherapy that’s calming and soothing like Lavender or Clary Sage. For the worms, and follow up with medical advice, an enema to cleanse the colon and/or a colon cleanse herbal kit.
